The following Student Forms should be submitted via FACTS SIS Family Portal

• Extended Absence Request Form
• Medication Authorization Form
• Allergy Action Plan Form
• Student Driver Permission Form
• Student Withdrawl Form
• Transcript Request Form

To complete the form, you should:

1. Log into your FACTS Family Portal.

2. Select SCHOOL on the left hand menu.

3. Select WEB FORMS.

4. Select FAMILY/STUDENT FORMS

5. Select desired form.

6. Complete the form and save.
