How NON-South Carolina students receive a SC Immunization Certificate:

Option 1: Call your doctor and ask them for the Immunizations on the SC form.   

Option 2: Call DEHC at 803-909-7300 and press option 0.   Let them know you are needing to transfer your out of state immunizations to the SC Form for Schools.  They will ask you for your email.   

Option 3: Visit DEHC office at SC DEHC 1070 Heckle Blvd #307 Rock Hill, SC 29732 Provide DEHC with the information above
