Transcript Requests Procedures
CURRENT STUDENTS
Students/Parents must submit this form at least two weeks prior to needed deadline. Last minute Transcripts Requests will be sent as soon as possible but we cannot guarantee their prompt arrival.
Official Transcripts are generated on a yearly basis and we do not issue mid-year transcripts other than for the current Senior students who need second semester transcripts for college applications.
Current Senior Students, applying via the Common App, do not need to submit a Transcript Request because we send transcripts, including mid-year and final automatically via the Common Application. If your college has requested transcripts, please be sure they do not already have a copy via the Common App.
To complete the Transcript Request Form, parents should:
1. Log into your FACTS Family Portal.
2. Select SCHOOL on the left hand menu.
3. Select WEB FORMS.
4. Select FAMILY/STUDENT FORMS
5. Select desired form.
6. Complete the form and save.
You must submit a separate request for each school/company.
GRADUATES or FORMER STUDENTS

- Student’s First, Middle & Last Name
- Student’s Contact Email
- Student’s Graduation Year/Expected Graduation Year
- Student’s Date of Birth
- Describe reason for request
- Describe format in which transcripts should be sent
- List Name of school(s)/company receiving transcripts
- Include School(s)/Company mailing address where transcripts should be sent or incldue email(s) to where transcripts should be sent
- Incldue Phone number of school/company where transcripts should be sent
